This is a ne-of-a-kind handblown glass sculpture of a Forceps-nose Butterflyfish (Forcipiger flavissimus). The famous hawaiian name for this fish is "Lauwiliwilinukunukuoi'oi", which means "like the leaf of the lauwili tree with a nose like a bird". This piece is mounted on a clear base and stands 6" x 4.5" x 2".
